Responsibilities
- Provide compassionate care and support to patients in hospice settings, ensuring comfort and dignity during end-of-life transitions.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans that meet the physical, emotional, and spiritual needs of patients.
- Educate families about the hospice process, available services, and bereavement resources to assist them during this challenging time.
- Monitor patient conditions and adjust care plans as necessary to ensure optimal quality of life.
- Maintain accurate and timely documentation of patient care, treatments, and interactions in accordance with regulatory standards.
- Participate in ongoing training and professional development to stay current with best practices in hospice care.
Requirements
- Valid nursing or caregiving license, as applicable, with a background in hospice, palliative, or similar care settings preferred.
- Strong interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work collaboratively within a team while also demonstrating strong independent judgment and decision-making skills.
- Compassionate and empathetic demeanor with a commitment to providing high-quality, patient-centered care.
- Flexibility to work various shifts, including weekends and holidays, to meet patient needs.
- Experience with electronic health records (EHR) systems is a plus.
